The Salvation Army Toronto Grace Health Centre (TGHC) is a 119 bed facility located in downtown Toronto. TGHC is committed to providing compassionate, patient centered care and services (medically complex and specialized care) to individuals who require our help. Our values of dignity and respect have equipped us to be leaders in General Complex Continuing Care, Post Acute Care Rehabilitation and Palliative Care.

Our Team

At TGHC we create conditions that enhance confidence in clinical judgement and team work. The team includes: Physician, Nurses, Registered Dietitian, Chaplain, Occupational Therapist, Physiotherapist, Rehabilitation Assistant, Pharmacist, Pharmacy Technician, Therapeutic Recreationist, Respiratory Therapist, Chiropodist and Volunteers.

Empowering Nurses

Formalized preceptorship programs support new employees. The care delivery model that was implemented best fits the organization and the unique needs of the patients and families we serve. Nurses develop therapeutic relationships; they demonstrate personal accountability and effective communication.

Nurses are guided by the interprofessional principles of collaboration, leadership and evidence based best practices and processes.

Active participation in committees and councils is encouraged and extend throughout the organization – Nursing Advisory Council, Interprofessional Practice Council, Joint Occupational Health and Safety, Infection Control, Pharmacy and Therapeutics and sub committees of Quality, Safety and Risk.

Education and ongoing learning

TGHC fully supports and implements evidence based best practices. We promote a spirit of inquiry with just-in-time learning exchanges between nurses and practice leaders. Financial assistance is provided for workshops, conferences, seminars and specialty nursing certifications. In addition, assistance is provided for college and university courses.
